Loneliness has been reframed from a ‘social problem of old age’ into a major public health problem. This transformation has been generated by findings from observational studies of a relationship between loneliness and a range of negative health outcomes including dementia. From a public health perspective, key to evaluating the relationship between loneliness and dementia is examining how studies define and measure loneliness, the exposure variable, and dementia the outcome. If we are not consistently measuring these then building a body (...) of evidence for the negative health outcomes of loneliness is problematic. Three key criteria had to meet for studies to be included in our analysis. To test the proposition that loneliness is a cause of dementia we only included longitudinal studies. For inclusion studies had to measure loneliness at baseline, have samples free of dementia and assess dementia at follow up. We identified 11 papers published between 2000 and 2018 that meet these criteria. These studies included seven different countries and only one was specifically focused upon dementia: all other studies were cohort studies focused upon ageing and health and wellbeing. There was extensive heterogeneity in how studies measured loneliness and dementia and in the use of co-variates. Loneliness was measured by either self-rating scales or scales. Dementia was assessed by clinical tests, diagnostic/screening tools, cognitive function tests, and self-reported doctor diagnosis. Substantial variation in loneliness prevalence and dementia incidence. Six studies did not report a statistically significant relationship between loneliness and dementia. Significant excess risk of dementia among those who were lonely ranged from 15% to 64%. None of these studies are directly comparable as four different loneliness and dementia measures were used. We suggest that the evidence to support a relationship between loneliness and dementia is inconclusive largely because of methodological limitations of existing studies. If we wish to develop this evidence base, then using a consistent set of loneliness and dementia outcome measures in major longitudinal studies would be of benefit. (shrink)

The phenomenon and implications of stigma have been recognized across many contexts and in relation to many discrete issues or conditions. The notion of spatial stigma has been developed within stigma literature, although the importance and relevance of spatial stigma for rural places and rural people have been largely neglected. This is the case even within fields of inquiry like public and rural health, which are expansively tasked with addressing the socio-structural drivers of health inequalities. In this paper, we argue (...) that developing a better understanding of rural place stigma is critical for addressing contemporary patterns of spatial injustice and health inequalities affecting rural communities globally. Drawing on international literature and examples from the reported experiences of rurally living Australians and news and other media, we present an analysis highlighting the power in rural place stigma. In doing so, we build a case for the relevance and importance of interrogating rural place stigma, especially in the fields of public and rural health, for changing the conditions within—and the broader positioning of—the rural in the public and political landscapes. Internet counseling or psychotherapy involves a variety of activities such as psychoeducation, individual therapy, and automated self-help interventions delivered via the Internet. Although other professional societies such as the National Association of Social Workers, the American Counseling Association, and the National Board of Certified Counselors have tackled the issues of Internet counseling ethics head on, the American Psychological Association has been conspicuously absent from this debate. Processes of rapid and truncated agrarian change—driven through expanding urbanisation, infrastructure development, extractive industries, and commodity crops—are shaping the livelihood opportunities and aspirations of Indonesia’s rural youth. This study describes the everyday experiences of youth as they navigate the changing character of agriculture, aquaculture, and fishing livelihoods across gender, class, and generation. Drawing on qualitative field research conducted in the Maros District of South Sulawesi, we examine young people’s experiences of agrarian change in a landscape of entangled rural, coastal and (...) increasingly urbanised spaces. We find that young people aspire to secure, modern, and salary-based work, while continuing to seek and sustain intergenerational farming or aquaculture-based livelihoods. Youth take advantage of increased connectivity to diversify their incomes, yet their dependence on mobility also introduces new forms of gendered and class based precarity such as insecure working arrangements, disruption to education and violence (especially for young unskilled women and youth from financially insecure households). Our study highlights the persistent conditions of precarity that many young people encounter in both rural and urban settings, while challenging assumptions that youth are uninterested in rural futures. The problem for many troubled youths seeking help within a Christian context is that their need for meaningful connections and spiritual growth is attached to relationships with their significant others. When needs of attachment are not adequately met due to the effect of an insecure attachment working model in a relationship with God, the teen may end up leaving the faith community seeking a new caregiver or regress into spiritual struggles, depression, anxiety, self-doubt and other negative emotions. This paper responds (...) to the issues associated with youth attachment behaviour towards God using a caregiving theology that draws on the 'power of two' through incarnational creativity and pneumatological ecclesiology. This proposed theology first supplements the work of the pastoral counsellor to create a transformative container through which young people, already within a redemptive Christian community, can grow spiritually and deepen their relationship with God. Secondly, pastoral juxtaposition targeting the spiritual needs of hurting youths serves to create a magnet space within the church community that models incarnational love, pastoral care and support for young people by entering their world order to reframe their personal stories of grief, abandonment and insecurity through creative means. Finally, we theologise that pastoral juxtaposition also integrates the power of the Holy Spirit, as the ultimate caregiver in times of conflict, into each youth's unique journey of faith and healing. Implications of this pastoral theology are discussed and applied to everyday life. (shrink)

Despite a growing literature on environmental inaction, it remains poorly understood. This article examines much of this literature including environmental ethics, policy studies, disaster theory, and psychology. Among the many existing explanations, we examine shifting values, rational incentives, and psychological barriers to action. Ultimately, we show how most of these explanations rely on simplistic assumptions about subjectivity. To address this, we apply the phenomenology of Maurice Merleau-Ponty to show how an understanding of habit informed by Merleau-Ponty’s phenomenology reveals the deeper (...) conditions of both action and inaction. The latter requires cognizers form not just a causal representation of the world but also of their own boundary and relationship with it, in order to diagnose the consequences of their actions. We suggest this locates the blanket in the eye of the beholder.

In this article, we present an ethics framework for health practice in humanitarian and development work: the ethics of engaged presence. The ethics of engaged presence framework aims to articulate in a systematic fashion approaches and orientations that support the engagement of expatriate health care professionals in ways that align with diverse obligations and responsibilities, and promote respectful and effective action and relationships. Drawn from a range of sources, the framework provides a vocabulary and narrative structure for examining the moral (...) dimensions of providing development or humanitarian health assistance to individuals and communities, and working with and alongside local and international actors. The elements also help minimize or avoid certain miscalculations and harms. Emphasis is placed on the shared humanity of those who provide and those who receive assistance, acknowledgement of limits and risks related to the contributions of expatriate health care professionals, and the importance of providing skillful and relevant assistance. These elements articulate a moral posture for expatriate health care professionals that contributes to orienting the practice of clinicians in ways that reflect respect, humility, and solidarity. Health care professionals whose understanding and actions are consistent with the ethics of engaged presence will be oriented toward introspection and reflective practice and toward developing, sustaining and promoting collaborative partnerships.
